Japanese Dentists Get Poor Remuneration at an Endodontic Treatment

Last time, I wrote about root canal irrigation and its importance. After that, I wanted to write something else about root canal irrigation in Japan.

I think, it seems that the step of root canal cleaning is not considered too important among Japanese dentists.

It seems that a combination of it and sodium hypochlorite solution is still being used in Japan. The number of doctors who use EDTA as irrigants for root canal cleaning may still be in the minority. It just is said that we are still behind the global standard of endodontics.

However, I feel that few doctors are interested in root canal treatment. I think one of the main reasons is the low remuneration for root canal treatment.


For example, the above image is my clinical case of root canal obturation at #42 and then I got about 40$ a remuneration. I always wonder if the remuneration for root canal treatment is a little low. In Japan, we have national health insurance, which means that patients pay much less at the counter.

 An important aspect of root canal cleaning is just disinfection, especially at the root apex. This consists of removal of the smear layer of the dentinal wall and disinfection with sodium hypochlorite solution in the apical delta. Normally, dentists perform positive pressure irrigation using a syringe for root canal irrigation. They use saline solution, hydrogen peroxide solution, EDTA, or sodium hypochlorite solution as the cleaning solution. If the needle is thin enough to rinse deep into the root canal, dentists will usually feel that can clean the root canal. However, the reality is that the cleaning is not satisfactory. In my opinion, an extremely important step in root canal treatment is root canal irrigation. Cleaning the inside of the root canal is a more difficult procedure than the dentist may think and, in practice, often results in inadequate cleaning. Root canal irrigation is not as simple as simply placing fluid into the root canal. As the name "irrigation" implies, it is the act of physically and chemically removing the dirt inside the root canal using a cleaning solution - some kind of liquid.
